This form is to help the Area Training Manager to plan courses which Leaders need.

You should have completed Module 2, your Personal Learning Plan, after talking it through with your Training Advisor or Local Training Manager before filling in this form, as this is the only way you will know which courses you need to do.

Fill in the form and click on 'Submit' and we'll try and take your needs into account when planning the next training programme, and contact you when a course you've requested is running.
